Reporting is open to all healthcare workers, including:
- Travel nurses across all specialties (ICU, ER, OR, med-surg, and more)
- Therapy travelers, such as traveling physical therapists and occupational therapists
- Allied clinicians working through travel therapy companies or allied medical staffing agencies
Part of what travel nurses do is advocate for patients. No matter the type of travel nurse or allied clinician, you can submit a safety concern directly.
How to Report a Patient Safety Event
Step 1: Gather details
Include:
- Facility name and address
- Date and time of the event
- Service or unit involved
- Description of what happened
Step 2: Submit online (preferred)
Use The Joint Commission’s Online Submission Form to file your report. You can submit anonymously. You can also visit their website directly at www.jointcommission.org.
Step 3: Mail your report (optional)
If you prefer, mail your report to:

Step 4: Notify your recruiter
After filing, let your travel nurse recruiter know so they can document the event and support you through any follow-up.
What Not to Include
Do not send medical records, photos, or billing information. The Joint Commission does not review individual medical cases or financial disputes, and these documents will not be accepted.
Non-Clinical Concerns
For issues unrelated to patient care, such as nurse housing, travel nurse accommodations, or rental arrangements, contact your recruiter or housing provider directly. These are not handled through The Joint Commission’s reporting process.
To protect your Personal Health Information, The Joint Commission will not accept copies of medical records, photos, billing invoices, or other related personal information. Any such document received will be shredded upon receipt, per our policy.
