FlexCare 360 User Feedback Survey (December 2023) | Official Contest Rules

1. Introduction: FlexCare Medical Staffing is sponsoring a giveaway, hereafter known as “Contest,” for an opportunity to win one (1) of two (2) two-hundred and fifty dollar ($250.00) Southwest Airlines gift cards. The Contest is only open to legal United States residents. Entries originating from any other jurisdiction are not eligible. This Contest is governed exclusively by the laws of the United States. NO PURCHASE IS NECESSARY.

2. Eligibility: Entrant must be 18 years or older and a legal resident of the United States. Entrant must be a registered clinician, be an active registered user of the FlexCare 360 mobile app, and complete the “FlexCare 360 User Feedback Survey.” Void outside of the United States and where prohibited, taxed, or restricted by law. All federal, state, and local rules and regulations apply.

3. Contest Period: Eligible entrants can enter to win the Contest one (1) time on December 22, 2023. The Contest will close at 12:00:00 p.m. PST on December 21, 2023.

4. How to Enter: To enter, entrants must complete the “FlexCare 360 User Feedback Survey.” Only one (1) entry per person is permitted. Each “FlexCare 360 User Feedback Survey” actually completed and received by FlexCare Medical Staffing will count as one (1) entry. Entrants submitting entries via automated entry devices or programs will be disqualified. FlexCare Medical Staffing is not responsible for lost, late, illegible, stolen, incomplete, invalid, unintelligible, misdirected, technically corrupted, or garbled entries, which will be disqualified, or for problems of any kind, whether mechanical, human, or electronic. Proof of submission will not be deemed to be proof of entry in the Contest. Alternatively, entry can be made by sending an email to [email protected] with the subject line, “FlexCare 360 User Feedback Survey”. Email entries must be received by the end of the Contest period to be eligible. Email entries are limited to one (1) entry per eligible health professional. 

5. Drawing: Winner will be selected on December 22, 2023 in a random drawing from all eligible entries, by FlexCare Medical Staffing, the decisions of which are final and binding in all matters relating to the Contest. Odds of winning depend on the number of eligible entries received. The potential winner will be notified by email or phone call (“Notification”) by 12:00 p.m. PST on December 22, 2023. The potential winner will have three (3) business days from Notification to claim their prize. Each entrant selected as a potential winner must comply with all terms and conditions set forth in these Official Rules and winning is contingent upon fulfilling all such requirements. If prize or prize notification is returned as undeliverable, if winner does not claim the prize within three (3) business days after Notification or rejects the prize, or in the event of non-compliance with these Contest rules and requirements, such prize will be forfeited, and an alternate winner will be selected from all remaining eligible entries. No compensation will be given for prize forfeiture.

6. Prizes: Each winner will receive one (1) Southwest Airlines gift card in the amount of two-hundred and fifty dollars ($250.00). No substitutions of prize by the winner are permitted, and FlexCare Medical Staffing reserves the right to substitute a prize of equal value due to prize unavailability. Prizes are not redeemable by the prize recipient for cash value. All taxes, fees, and surcharges on prizes are the sole responsibility of the winner. Prize will be delivered via email to the email address provided by the winner.

7. Conditions: By participating, entrants and winner(s) agree to release and hold harmless FlexCare Medical Staffing, its subsidiaries, affiliates, advertising and promotions agencies, partners, representatives, agents, successors, assigns, employees, officers, and directors from any and all liability for loss, harm, damage, injury, cost, or expense whatsoever including, without limitation, property damage, personal injury, and/or death that may occur in connection with or participation in Contest, or possession, acceptance, and/or use or misuse of prize or participation in any Contest-related activity, and for any claims based on publicity rights, defamation, invasion of privacy, and prize delivery.

8. Miscellaneous: A potential winner may be requested to provide FlexCare Medical Staffing with proof that the potential winner is the winning entrant by proof of government or state-issued I.D. showing the full name of the potential winner. Additionally, the referred registered clinician must provide license verification before the potential winner is awarded the prize. FlexCare Medical Staffing assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft, or destruction of, or unauthorized access to, or alteration of, entries. FlexCare Medical Staffing is not responsible for any problems or technical malfunction of any telephone network or lines, computer online systems, servers, or providers, computer equipment, software, failure of any email or entry to be received by FlexCare Medical Staffing on account of technical problems or traffic congestion on the Internet or at any website, any combination thereof, or otherwise, including any injury or damage to entrant’s or any other person’s computer related to or resulting from participation or downloading any materials in the Contest. FlexCare Medical Staffing may prohibit an entrant from participating in the Contest or winning a prize if, in its sole discretion, it determines that said entrant is attempting to undermine the legitimate operation of the Contest by cheating, hacking, deception, or other unfair playing practices (including the use of automated quick entry programs) or intending to annoy, abuse, threaten, or harass any other entrants or FlexCare Medical Staffing representatives.

  • CAUTION: ANY ATTEMPT BY AN ENTRANT TO DELIBERATELY DAMAGE ANY WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE CONTEST MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAWS, AND SHOULD SUCH AN ATTEMPT BE MADE, FLEXCARE MEDICAL STAFFING RESERVES THE RIGHT TO DISQUALIFY THE ENTRANT FROM THE CONTEST AND SEEK DAMAGES FROM SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW.

9. Use of Data: FlexCare Medical Staffing will be collecting personal data about entrants in accordance with its privacy policy. Please review our privacy policy here. BY PARTICIPATING IN THE CONTEST, ENTRANTS HEREBY AGREE TO OUR COLLECTION AND USAGE OF THEIR PERSONAL INFORMATION AND ACKNOWLEDGE THAT THEY HAVE READ AND ACCEPTED OUR PRIVACY POLICY.
